If you were claimed as a dependent on your parent's tax forms, you must also submit a copy of your parents' 2023 Federal Income Tax Return.
4. A copy of your 2024-2025 FAFSA Submission Summary Report
You are required to submit a copy of the 2023-2024 Free Application for Federal Student Aid (FAFSA) Submission Summary Report with your application, available from https://studentaid.gov/h/apply-for-aid/fafsa. Recommenders must submit their letters directly to writers@uclaextension.edu, attention: Gebauer Recommendation.
